I backport modcache v1.4.2 to lighttpd 1.4.18 as modcache v1.4.3

Changelog:

I update examples to demonstrate how mod_cache works with mod_flv_streaming.

I think it works for lighttpd 1.4.19 with slighty modification to patch

hi 有2个问题请教
1.mod_mem_cache和mod_cache能不能一起用，我通过改patch把2个都装上了，但是不知道mod_mem_cache有没有日志，看不出来是不是有问题。
2.mod_cache支不支持限定cache在文件系统中占用的总空间？

1) mod_mem_cache 可以和 mod_cache 同时使用
2) mod_cache 还不支持限定cache在文件系统的总空间
